After your New Patient Free Consult you can schedule your first visit. 
At this appointment I will review your medical history, ask about your health status "from head to toe", and perform a physical exam that will include some functional medicine testing. I will then provide you with instructions to get lab work or other studies done to help me better understand your healthcare needs. A detailed Treatment Plan will be sent shortly after the appointment with additional instructions.
Approximately 90 minutes.
   *If during the Prospective patient consultation it is determined that your medical history warrants a more extensive evaluation, we will discuss this and I will adjust the appointment length and cost accordingly.

Schedule an Initial Report of Findings appointment.

Meet with me to discuss your initial lab results over a telemedicine video, or in-person if requested.
I will review in detail with you my report of findings and integrate your lab results into a revised Treatment Plan. Any prescriptions or supplement recommendations will be included and new follow-up instructions will be given.

Second official visit, after the Intake and Exam, In-Person.

At this appointment we will review the initial Report of Findings for any Lab work completed thus far. The Treatment Plan will be advanced to include recommendations for any prescriptions or supplements and follow-up instructions. Up to 60 minutes.

Periodic laboratory testing is necessary to track your metabolic response to treatment.
At this telemedicine appointment we will review the test results and adjust your treatment plan as needed.
Approximately 60 minutes.
  *The cost may be adjusted if the appointment takes less than 45 minutes or lasts longer than 90 minutes.

Just need a simple evaluation without becoming an active patient?

This focused history and examination is for those interested in obtaining medical clearance for school sports programs, or for enrollment in an exercise, weight-loss, or detoxification program. EKG interpretation (if necessary) and signed paperwork included.
Approximately 60 minutes.

I have not seen you for a physical examination in quite a while and I want to make sure that we are addressing all of your healthcare needs.
