Your first step is a provider consultation.
 

Your consultation package includes:
 

1. Provider Review & Consultation
Complete your intake forms before your visit. Your provider reviews your information and meets with you virtually to discuss your health history, symptoms, and goals.
 

2. Personalized Lab Planning
If updated labs are recommended, your provider will customize your lab panel based on your evaluation
 

3. Results & Treatment Planning
After your results are received, you will schedule one included follow-up visit to review your labs and discuss treatment options when clinically appropriate.
 

4. Treatment & Ongoing Care
If treatment is prescribed, medications will be shipped directly to your home through a licensed pharmacy, with follow-up and monitoring as needed.

Separate costs may include:

• Lab testing and related collection or shipping fees, reviewed before your order is placed
• Medications
• Ongoing care plans 

Important: 

• Prime Form Care currently provides telehealth services only to patients who are physically located in Virginia at the time of their visit. You do not need to be a Virginia resident, but you must be physically present in Virginia during your telehealth appointment.
• Prescriptions, lab orders, diagnoses, and treatment plans are not guaranteed. Controlled medications, including testosterone, may only be prescribed when all applicable requirements are met.
• Consultation fees become non-refundable once provider review, clinical communication, or the scheduled consultation process has started. Missed appointments or cancellations with less than 24 hours’ notice are subject to a $75 fee.
