A $100 booking deposit is required at the time of scheduling and will be applied toward your balance due at the time of treatment.

A current Good Faith Estimate (GFE) is required and will be provided prior to treatment. If you have not completed a GFE within the current calendar year, one will be required before your appointment and may be applied toward your total treatment cost as applicable.
