Thank you for booking your appointment with UHA Medical Center! Information to keep in mind:

• Appointment Details: Your appointment is an agreement between you and the clinic for a reserved time for services, if you need to cancel, please allow 24 hours notice. If you do not show, and no  notification given within 24 hours, we reserve the right to charge the card on file.  A $50 fee will be charged if you miss your appointment without prior notice. We realize that there are emergencies and this rule and determination to charge is up to the discretion of the owner depending on the circumstance. 
• Payment: A valid credit card has been saved on file for your convenience. Your card will not be charged until after your visit is complete.
