Follow-up visits focus on reviewing progress, managing chronic conditions, adjusting medications, reviewing lab results, and addressing new or persistent concerns. Visits remain unrushed and personalized.

May include:
Review of symptoms or treatment response
Chronic condition management
Medication adjustments or refills
Lab or imaging review
Preventive care follow-up

This comprehensive visit establishes care with Elite Health & Wellness under our Direct Primary Care (DPC) model.

During this appointment, we will review your complete medical history, current concerns, medications, lifestyle, and health goals. You’ll have unrushed time with your provider to address acute or chronic issues, discuss preventive care, and create a personalized care plan.

May include:
Complete medical history review
Focused or comprehensive physical exam (as appropriate)
Review of acute and chronic conditions
Preventive care and wellness planning
Medication review and management
Lab review or lab ordering (if indicated)
Care coordination and follow-up planning

Direct Primary Care membership is required or will be initiated at this visit.
