Appointment & Cancellation Policy

• Proficient Health & Wellness is a cash-based clinic and does not bill insurance.

• A valid card on file is required to book any appointment.

• Appointments must be cancelled or rescheduled at least 48 business hours in advance.

• Late cancellations, no-shows, or arrivals over 15 minutes late may result in a fee charged to the card on file.

• All required forms must be completed before your appointment or it will be rescheduled.

By booking, you authorize applicable charges per clinic policy.
