To schedule a botox party for groups of 3-6 guests
1. Register as a new patient by clicking this link. Then Create a patient portal account, complete the required forms, and send a portal message requesting a botox party.
2. Returning patients can simply send a portal message requesting a botox party.
3. Please specify day/date and AM/PM preference, and Revivo Concierge Aesthetics will respond with the closest available options.
4. Please specify in your message if you are the host, and how many guests will be attending.
5. Please have your guests follow the above directions to create an OptiMantra account, and send a message as well, clarifying the host name & date/time of the party they are joining.
6. Once successfully scheduled, each guest will receive forms to be completed prior to the party.
A follow up visit is encouraged 10-14 days after your first visit for Botox injections with Revivo Concierge Aesthetics. Enhancements can be made if necessary, and result photos will be documented. $17/unit
