$100 - Initial Fee which includes one Initial Visit and a Follow-Up visit in a One Month period.

Plan B - Three Months Urgent Care Plan 30 Min $ 200.00

$200 - Initial Fee / with monthly follow-ups for the following three months.

Plan C - Six Months Urgent Care Plan 30 Min $ 300.00

$300 - Initial Fee which includes one Initial Visit and a monthly Follow-Up for a Six Month Period.

Plan D - Eight Months Urgent Care Plan 30 Min $ 400.00

$400 - Initial Fee which includes one Initial Visit and a monthly Follow-Up for an Eight Month Period.

Plan E - One Year Urgent Care Plan 30 Min $ 600.00

$600 - Initial Fee which includes one Initial Visit and a monthly Follow-Up for a One Year period.

Plan Members - Urgent Care Visit / Follow-Up Visit 30 Min $ 0.00

This service is exclusively available to Plan Members. Please ensure you are enrolled in a Plan before booking this service, otherwise your appointment will be cancelled.

PLAN MEMBERS:

Plan A: One follow-up appointment available.
Plan B: One follow-up appointment per month for a period of three months.
Plan C: One follow-up appointment per month for a period of six months.
Plan D: One follow-up appointment per month for a period of eight months.
Plan E: One follow-up appointment per month for a period of twelve months.
