Important: Please make sure you are scheduled with a provider who is licensed in the state where you will be physically located during your appointment. By law, providers can only see patients who are physically present in a state where they hold an active license at the time of the visit.
If you are scheduled with a provider who is not licensed in your state, your appointment cannot take place and will need to be canceled and rescheduled with a provider who is licensed in your state.
*If you are booking for your child, they need to be present for a few minutes on screen for a quick physical exam. The rest of the appointment can be done with the parent.
Please note: All appointments are telehealth/virtual. You will receive a link to join a video call in your email.
Card on File: You will be asked to put a credit card on file. You will NOT be charged to book an appointment. If using insurance that we accept, your insurance will be billed.
CANCELLATION AND NO SHOW POLICY All appointment cancellations or changes within 24 hours of your scheduled appointment will incur a $50 fee. All No Shows will incur a $100 fee.
