Disclaimer:  This is a telemedicine practice for urgent care and medical weight loss.  This is not a primary care practice that does chronic disease management at this time.  This practice is DPC or concierge practice that does not accept insurance and the patient will be responsible for any medical services rendered. 

The medical fees for each visit must be paid at the time of the appointment and you will need to have a form of payment at the time of booking but you will not be charged until the appointment is complete.  You can cancel within 24-48 hrs prior to your appointment but if you do not cancel within 24-48 hours or do not answer or join the phone or visit over 5-10 minutes, you will charged a $25 no show/late/cancellation fee.   If your form of payment is declined, you will be charged any additional $50 fee and if you do not pay the invoice sent within 24 hours, you will be discharged from the practice if payment arrangements are not made within 24-48 hours and your outstanding bill is not paid within 1 week.  

If the provider orders any labs, you will be notified of the additional cost for any labs at the time of the appointment and you will receive a lab requistion form.  Your form of payment will be charged for the visit, medications/prescriptions, and labs at the time of the visit.  You will receive a receipt or superbill at the time of the encounter or visit.

Urgent Care Appointments:
The urgent care visits are 15-30 minutes in duration.  Due to the limitations of telemedicine, you can only book appointments for the following medical conditions:  upper respiratory infections (colds, acute bronchitis), pharyngitis (sore throat), sinus infections, COVID infection, urinary tract or bladder infections, or headaches. 

You can NOT book any appointments for the any of the following serious medical conditions:  chest pain, shortness of breath, stroke-like symptoms (e.g. trouble speaking, trouble swallowing, visual changes, or weakness or paralysis on one side of the body or face), bleeding, cuts or abrasions that need First-Aid or suturing, wounds that need wound care, head injuries, or severe abdominal pain. 

If you book an appointment for any of these serious, potentially fatal medical conditions, you will be directed to seek medical attention with a provider where you can have a face-to face encounter like urgent care or emergency room and your appointment will be cancelled. 

All consent and intake forms MUST be filled out 24 hours prior to the appointment.  If it is a same day appointment, all forms must be filled out at least 1 hour prior to the visit.  

Medical Weight Loss Appointments:
The medical weight loss visits are 30 minutes initially and 20 minutes for follow-up visits.  All consent and intake forms must be filled out at least 24 hours prior to the visit.  If you book a medical weight loss visit, you can cancel within 24-48 hours.  However, if you cancel less than 24-48 hours or the same day or your appointment 5-10 minutes late, you will incur a $50 no show/late/cancellation fee.  No exceptions.  

At the end of the initial and follow up appointments, you will receive an after visit summary that will outline the diagnosis, plan of care or treatment plan, any prescriptions or labs ordered, and any follow up instructions.

Prescriptions for GLP-1 agonists like terazipatide (Zepbound) and semaglutide (Wegovy) are electronically prescribed to either Empower or Southend pharmacy and the prescriptions are shipped to the patients directly within 3-5 days to the preferred mailing address on file.  You will pay for the manufacturer cost of the medication and refills will be available upon request.  Each time a refill is requested, you will be billed for the prescription on the form of payment provided.  You will receive a tracking email or text message once the prescription is shipped.

What is the best use by date?

The best use by date (BUD) is the recommended date or time frame when a medication or food is at its best quality per the FDA. 

The best use by date (BUD) for Southend Pharmacy is 84 days so 3-month and 1-month shipments are available.  The BUD for Empower is 28 days from when used or the BUD whichever is first.

What are the shipping costs?

If you have any questions or concerns after your visit, you can communicate through the patient portal to maintain and protect your privacy.  You can also call the office and leave a message.  

Any labs will be discussed at the visit or upcoming visits.  If there are any abnormal labs that require medical attention, you will be notified by the provider directly or the staff with the provider's approval.  The provider may also address lab results through the portal.
