Practice Policies

Phoenix Health Wellness & Aesthetics is a physician-led self-pay wellness practice focused on medical weight loss, metabolic wellness, virtual urgent care, and non-invasive aesthetic services.
 

Please note:
• We are a self-pay model at this time.
• Payment is required prior to or at the time of service.
• A valid payment method is required to reserve appointments.
• Membership services require active enrollment and recurring payment authorization.
• Patients may cancel or reschedule appointments with at least 24 hours.

Late cancellations, missed appointments, or repeated no-shows will be subject to cancellation fees or scheduling restrictions.

This practice does not provide emergency medical services. If you are experiencing a medical emergency, please call 911 or seek immediate emergency care.

By scheduling an appointment, patients acknowledge financial responsibility for services rendered.
 

Primary Care / DPC Membership


Establish ongoing primary care through Primary Care membership model.  Membership is designed for patients who want accessible, relationship-based care with ongoing chronic disease management, preventive care, medication management, and convenient access to their physician.

Established Patient Follow-Up

For current Phoenix Medical Spa PLLC patients who have already completed their initial consultation or established care with the practice.  These appointments are used for follow-up of an existing treatment plan, medication management, chronic condition follow-up, medical weight management follow-up, lab review, or other previously established care.

If you are a new patient and have not yet completed an initial consultation or established primary care, please select the appropriate new-patient service instead.

 

Initial medical weight loss consultations are 45-60 minutes. Follow-up wellness visits are typically 20–30 minutes depending on treatment needs.

All intake and consent forms must be completed prior to the appointment. A valid payment method is required to reserve appointments.

Appointments canceled with less than 24 hours notice, missed appointments, or late arrivals greater than 10 minutes may be subject to a cancellation fee.

After your visit, you may receive a personalized treatment plan, wellness recommendations, prescriptions when appropriate, and/or laboratory recommendations.

Virtual Urgent Care

Virtual urgent care visits are intended for non-emergency medical concerns including:

• cold and flu symptoms
• sinus infections
• sore throat
• allergies
• urinary tract infections
• rashes
• headaches
• other minor acute illnesses

If you are experiencing chest pain, severe shortness of breath, stroke-like symptoms, severe abdominal pain, uncontrolled bleeding, or another medical emergency, please call 911 or seek immediate emergency care.

Important Practice Information

Phoenix Medical Spa is a self-pay physician-led wellness practice. We do not accept insurance at this time but are in the process of being credentialed.

Membership services may require an initial consultation and medical evaluation prior to enrollment.

Phoenix Medical Spa currently provides telemedicine services to patients located in the following states at the time of their appointment:

Arizona, California, Florida, Georgia, Idaho, Illinois, Indiana, Iowa, Montana, Nebraska, New York, North Carolina, Ohio, Oregon, Texas, Virginia, Washington, and Wisconsin.

Patients must be physically located within a state in which Dr. Hooks is licensed at the time of the telemedicine visit in order to receive services.

Additional fees may apply for laboratory testing, prescription medications, supplements, or outside services recommended during treatment.

Patients are financially responsible for services rendered, laboratory testing, and prescribed medications.
