Important Booking Information

  • Forms: Intake and consent forms must be submitted online at least 48 hours before your appointment. Appointments may be canceled if forms are not submitted on time.
     

  • Insurance: We accept in-network insurance from Mountain Health Co-Op, BlueCross BlueShield, Allegiance, Cigna, Pacific Source, First Choice Health, and Aetna. For other insurance, we are out-of-network but can provide a superbill for reimbursement. Always check eligibility directly with your insurance provider prior to the appointment.
     

  • Not Covered: Medicare, Tri-Care/Veteran, and MT Medicaid do not cover naturopathic care.
     

  • Payment: If out-of-network, full payment is due at the appointment. Fees are $300 per hour, plus $50 for every 15 minutes over. New patient visits are approximately $300–$350, and follow-up appointments are $150–$270 (30–54 minutes). Lab fees vary depending on lab and service.
     


In-Person Follow-Up Visit (Adult/Child)

Duration: 30–45 minutes


Please note in the “Reason for Visit or Comments” section: follow-up for medical diagnosis/lab review.


Follow-up visits for adults and children typically last 30 minutes and are intended to discuss medical diagnosis(es), review lab or procedure results, and update your wellness plan. Depending on complexity, the visit may last up to 45 minutes.

Telemedicine Follow-Up Visit (Adult/Child)
Duration: 30–45 minutes

New Patient Appointment (Adult/Child) – In Person
Duration: 60 minutes

This initial visit is required to establish care and includes a thorough review of medical history, a comprehensive physical exam, and discussion of an individualized wellness plan.


New patients must be seen and establish care before any medical advice, treatment recommendations, or laboratory testing can be ordered.

DOT Physical – In-Person
Fee: $100 (Full payment due at appointment)


A Department of Transportation (DOT) physical is required for individuals who drive commercial vehicles. The purpose of this exam is to ensure that drivers are physically, mentally, and emotionally fit to operate commercial motor vehicles safely.


Please bring to your appointment:
  1. Valid State Driver’s License
  2. List of current medications
  3. Glasses or contacts, if you use them

  5. Be well-hydrated to provide a urine sample
