Insurance:
Quantum Women's Health is an out-of-network specialty practice. We do not accept or bill insurance, and payment is due at the time of service. We do provide superbills that you may submit to your insurance plan for potential out-of-network reimbursement. 
 

Cancellation & Rescheduling Policy:
We require 48 business hours notice for all cancellations or rescheduling. Changes made in less than 48 business hours will be charged 50% of the appointment fee. Please call 208-510-0261 to cancel or reschedule your appointment.
