Scheduling & Payment Policy

At Freeman Wellness and Aesthetics, we require full payment for the visit upfront at the time of scheduling the appointment.
Patients can change their appointment date and time up to 24 hours before the scheduled appointment.
After the 24-hour mark, rescheduling will no longer be possible, and the payment will be forfeited.
Patients who are more than 15 minutes late for their scheduled visit will have their appointment considered canceled and will forfeit their payment.

By scheduling and paying for this visit you are agreeing that you understand the above statements and will adhere to them.

Patients wishing to establish care for routine primary care visits, medication management, chronic disease management, labs, tests/imaging.
(Visit time: 45 minutes - 1 hour)

While the VA makes the final decision on disability ratings, the strength of your medical evidence you submit can make all the difference. We take the time to thoroughly document your service history, health concerns, and daily limitations — details that can sometimes feel overlooked or rushed in standard evaluations. Our goal is to ensure your service history, health concerns, and limitations are thoroughly documented and not overlooked.

$800 Flat Fee
For $800, you receive a comprehensive evaluation and documentation set that typically costs $1,200–$1,500 elsewhere.
*Additional DBQs are available at $50 each, so your cost only increases if you are filing multiple conditions beyond the standard two.
