This brief introductory consultation is designed for individuals who have questions about our services or want to determine whether Elevare Wellness is the right fit for their health goals. 

This visit does not include medial diagnosis, prescribing, or treatment. 

If you proceed with an Initial Comprehensive Evaluation within 14 days of this consult, the full $59 consult fee will be credited toward that visit. 

This credit is non-refundable, non-transferable, and expires 14 days after the consult. 

This visit includes a comprehensive medical evaluation to determine candidacy for services offered through Elevare Wellness. 

Treatment options may include:
• Weight optimization therapy (including GLP-1 receptor agonists such as semaglutide or tirzepatide when clinically appropriate)
• Peptide therapy
• Bioidentical hormone replacement therapy
• Metabolic and wellness optimization programs

Evaluation includes:
• Focused medical history review
• Review of available laboratory results (if provided)
• Risk assessment and contraindication screening
• Discussion of appropriate treatment options
• Development of a personalized care plan

Laboratory testing is not included and may be ordered separately based on medical need. Labs may be completed using insurance or cash-pay options.

The Initial Comprehensive Evaluation fee is $199. Patients who choose to move forward with a 12-week program will receive a $99 credit toward their program cost.
