This brief introductory consultation is designed for individuals who have questions about our services or want to determine whether Elevare Wellness is the right fit for their health goals. 

This visit does ot include medial diagnosis, prescribing, or treatment. 

If you proceed with an Initial Comprehensive Evaluation within 14 days of this consult, the full $59 consult fee will be credited toward that visit. 

This credit is non-refundable, non-tranferable, and expires 14 days after the consult. 

This visit is for current patients enrolled in a wellness program who require ongoing care, medication adjustments, lab reviews, or refill authorization. 

Regular follow-ups ensure safe, effective, and optimized treatment outcomes. 

If you are new to the practice, please schedule the Initial Comprehensive Evaluation instead. 

This comprehensive telehealth medical evaluation includes a detailed review of your health history, required laboratory results, and personalized treatment recommendations. 

Completion of required labs prior to your appointment is mandatory. Appointments without completed labs will be rescheduled per clinic policy.
