Lab appointment - Non-Synergy Patients, $50 $ 50.00
This is a 15 minute lab appointment. Please arrive well hydrated. If you were given a lab kit, please bring with you with (2) frozen ice packs. Please be sure to fill out your consent for collection ahead of your appointment. Make sure you also have your printed order. 
Lab visit (complimentary phlebotomy) for Established Synergy patients $ 0.00
This is a lab draw appointment for our established patients. 
We have already ordered your labs; please make sure all fees are paid and lab order is ready (if applicable)

15 minute Strategy Call - Nutrition & Coaching Programs $ 0.00
This 15 to 20 minute complimentary Strategy Call is designed for patients interested in our  nutrition consults,  and our 12 week mind- body and 12 week gut health programs RN led coaching programs. 

During this call, you'll connect with Karen, RN,  to discuss your goals, current challenges, and the support you're looking for so we can help guide you toward the best next steps for your journey. 
You can also discuss the optional add on of lab testing to your coaching program. 

Please note: 
This call is not intended for hormone management, prescription discussions, lab review or medical care with Deziree- these would require a 2 part Foundational Assessment instead.
LAB VISIT - Mojo & Mens Metabolic Lab Panel $ 0.00
This lab draw is provided complimentary. Please arrive hydrated with water, fasting, and hold your supplements and medications until after your blood work is collected. Please arrive 5 minutes before your appointment. If you are late, we reserve the right to reschedule. ?
