To move forward with your appointment, you must be able to complete the following:
Complete all required intake forms in the patient portal
Upload requested documents (ID, insurance card, photos, and any recent labs)
Prepare to actively participate in your care
Appointments are not fully confirmed until all forms and uploads are completed.
If forms are not completed before your visit, your appointment may be delayed or rescheduled.

Virtual Visits
All appointments are conducted by telehealth. You will receive a secure video link after booking.
Office Hours
10 am to 4 pm, Wednesday, Thursday, and Friday. Saturdays available upon request. If you require a day and time that is not available, send your request to [email protected].
