Cuozzo Health offers VIRTUAL integrative medicine services for adults 18+ residing in CT.
In-person consults in Berlin, CT are considered on a case-by-case basis, for an additional fee. Please call the office.

IMPORTANT BOOKING INFORMATION:

NEW PATIENTS:
All prospective patients shall have a complimentary Discovery Call, prior to booking any services.
Initial Case Review: Consent/intake forms must be completed AT LEAST 5 DAYS prior to the appt, or you will be rescheduled
(deposit policy applies).

BOOKING:
Discovery Calls may be booked online below, or by calling the office.
All other services are booked by contacting the office.
A $50 deposit, credited toward the cost of the visit, is required for ALL non-package bookings EXCEPT Discovery Calls.
If unable to keep an appointment, cancel/reschedule by calling, emailing or messaging in portal, at least 48 hours in advance to avoid a $50 cancellation fee (deposit not credited back).
If you cancel with at least 48 hours advance notice, but cannot reschedule, the deposit will be refunded within 14 days.
The above policy is explained on all booking emails, and consent forms.

PAYMENT:
Direct Patient Care (DPC) pricing is a self-pay model. We accept cash (if in person), major credit cards, HSA/FSA (no checks or app payments).
Payment is due at time of service. No refunds/exhanges for any services.
The visit cost does not include costs for lab testing or supplements, unless otherwise stated.
You may submit a superbill to your insurance for possible reimbursement of all services.

This call is required prior to booking an Integrative Medicine Initial Case Review.

INITIAL Case Review (Virtual) 80 Min $ 475.00

THIS IS YOUR FIRST STEP ON YOUR HEALING JOURNEY TO WELLNESS!

An 80-minute Initial Case Review is required for all new patients, or those who have not had an integrative visit within the past 12 months.
Pricing includes one 15-minute LAB REVIEW phone call (if labs are ordered) once all results are back, to discuss the results and next steps.
*Enroll in a PACKAGE at your Initial Case Review and receive $50 off the cost of the package!

Note: A complimentary Discovery Call is required for all new patients prior to an Initial Case Review.

This VIRTUAL visit includes:

Up to 80 minutes of face-to-face time with Cheryl
Review of your health history, wellness goals, lifestyle habits and patterns
Initial personalized care plan (sent within 5 business days) to get you started on your journey
Lab tests ordered as needed
15-minute LAB REVIEW phone call (if labs are ordered) once all results are back
Cheryl's time spent (up to 4 hours outside of visit) reviewing, researching, and making evidence-based recommendations

30-minute Follow-Up (Virtual) 30 Min $ 150.00

THE NEXT STEPS ON YOUR HEALING JOURNEY TO WELLNESS!

Follow-up consultations are offered virtually for established patients who have already had an Initial Case Review and have been seen within the past 12 months.

This VIRTUAL visit includes:

30 minutes of face-to-face time with Cheryl
Discussion about how your wellness journey is going and any concerns or new issues
Follow up of your previous treatment plan and labs
Tweaking of your care plan in order to remove barriers to wellness
Further lab testing if recommended
Cheryl's time spent outside your visit (up to 2 hours) reviewing, researching, and making evidence-based recommendations

Please provide:

A list of all the medications and supplements you are currently taking

Copies of any other recent medical records you would like Cheryl to review

30-minute Follow-Up (Berlin) 30 Min $ 195.00

This Follow-Up Consult is the same as the above description, but takes place IN-PERSON at our office located at:

All Integrative Medicine Follow Up Consults will take place virtually unless otherwise prearranged.

Auricular Acupuncture 40 Min $ 65.00

This ear acupuncture and acupressure treatment involves gentle placement of up to 5 needles or seeds and beads to specific sites on each ear. The recipient then sits and relaxes quietly. Those receiving acupuncture find a sense of empowerment; that their body has the power to heal itself. Scared of needles? No worries! Tiny metal beads or seeds can be used on the same points as a form of acupressure. The beads and seeds can be left in place for up to one week. They give the same result at a slower pace.

Reduced cravings for alcohol and other drugs, including nicotine
Support for stress, PTSD/trauma and grief

Patients having in-person IM consults in the Berlin office who would like to add acupuncture to the visit, take $15.00 OFF this service! Must be scheduled in advance.

Cupping Therapy 40 Min $ 75.00

In cupping therapy, special cups are placed on the skin and air is pulled out to create a vacuum. This helps to aid circulation and lymphatic flow, which may help relieve muscle tightness or stiffness; reduce pain or swelling, or improve athletic recovery. Many patients find it to be the most relaxing and enjoyable part of their visit, and often fall asleep! 1-2 body parts may be treated per session.

Patients having in-person IM consults in the Berlin office who would like to add cupping to the visit, take $15.00 OFF this service! Must be scheduled in advance.

15-minute Lab Review Phone Call (AFTER INITIAL CASE REVIEW) 15 Min $ 0.00

This is a one-time booking included in the price of an Initial Case Review, only if labs were ordered, so that we can review the results once they are all available. This 15-minute call should be booked 10-14 days from the day you have your labs collected. We will wait until all results are available to have this call, unless there is a critically abnormal result that needs to be addressed immediately.

It is the patient's responsibility to ensure this call is scheduled and fulfilled. Please contact the office to schedule.

If there is an alternative number you would like us to call, please message us in the patient portal at least 24 hours prior to your appointment. *If there is no answer, the session is forfeited and can be rescheduled at the phone check-in rate of $110.00.
