Follow-up direct primary care visit: sick visit, symptom management, physical, general questions

This exploratory phone consult is a brief, introductory conversation between you and me, Amber Jirschele, NP, designed to discuss health concerns, goals, and potential approaches to care. During this phone consultation, you will briefly share your history, I will answer questions about functional medicine strategies and explain how a tailored plan may address underlying issues. This call helps determine whether the practice is a good fit for your needs and sets the stage for more personalized and comprehensive follow-up visits.

This is the first consultation. This 1-hour consultation offers a thorough assessment to start your personalized, medically supervised weight loss plan. It targets the underlying causes of weight gain, metabolism changes, or body composition issues unresponsive to diet and exercise, and creates an effective treatment strategy for you.

What to Expect:

• A deep dive into your health history, current symptoms, and lifestyle
• A review of lab results-if previously completed (hormones, thyroid, insulin resistance, nutrient status, etc.) to identify metabolic barriers to weight loss
• Education on weight loss medications (such as GLP-1s and other options), and how they may support your goals, or are counterintuitive to wellness
• A personalized treatment plan that may include prescription medication, nutrition, and movement strategies to support your goals.

This is the first consultation. The initial functional health appointment is a comprehensive evaluation aimed at understanding your unique health needs by focusing on root causes rather than just symptoms. It involves pre-appointment preparation with a detailed health questionnaire covering medical history, lifestyle, diet, and stress levels. The consultation includes a thorough discussion and assessment of medical history, lifestyle factors, current symptoms, and a physical examination. Previous lab and imaging review is included, and additional lab testing covering areas such as kidney function, electrolyte balance, and blood sugar levels will be recommended, giving us a solid foundation to assess your health status. Advanced diagnostic tests such as blood, allergy, and gut health tests may be recommended. We will collaborate to set health goals, develop a personalized plan, and establish milestones. Follow-up care is discussed to monitor progress and adjust the health plan as needed.
This visit is IN-PERSON only.

This first visit establishes your healthcare plan, with providers spending an hour reviewing your history, health goals, and performing a thorough assessment. The membership covers everyday health issues and concerns, chronic condition management, and lifestyle discussions.

The focus is on building a long-term provider relationship and ensuring proactive, transparent care.

While DPC isn’t insurance, many pair it with a high-deductible plan for major events. 
A flat monthly fee of $100 includes all primary care services, with significantly discounted lab work and imaging (with ability to utilize your health insurance for these services), 3 virtual visits and 3 in-person urgent health concern visits (per year) and no unexpected bills. This is a Membership service, billed in 3 month increments. Discounts will be given for yearly memberships, paid in advance.

During a one-hour anti-aging/peptide therapies consultation, clients receive a personalized overview of functional medicine strategies, including nutrient-dense diets, targeted supplements like NAD+ and other optimizing peptides, and lifestyle interventions such as sauna use, cold immersion, fasting, and exercise. These evidence-based approaches are tailored to optimize cellular resilience, promote longevity, and improve quality of life through integrated diagnostics and regenerative therapies. This visit includes one NAD+ Injection.You’ll have direct access to your provider via phone, text, or email, plus same- or next-day follow-ups/urgent health concerns visits.

This visit includes: B12 injection, follow up of how the hormone replacement therapy is doing, adjusting dosing based upon symptoms, and a prescription refill (sent to pharmacy or in office).  NO LABS ARE INCLUDED IN THIS VISIT.

The first in-person visit for HRT is essential for establishing care and ensuring safe treatment. Patients provide their medical history, complete questionnaires and complete consent forms prior to appointment , so the provider is very familiar with the reason for visit. Baseline lab tests may be ordered to check hormone and organ function at, before, or after this appointment.

This one-hour consultation offers a comprehensive, individualized assessment to facilitate the initiation of hormone replacement therapy (HRT). It is intended for individuals experiencing perimenopause, menopause, or hormone-related symptoms—such as fatigue, cognitive changes, mood fluctuations, decreased libido, or sleep disturbances—ensuring a thorough understanding and supportive environment throughout the process.

Visit Steps

• Comprehensive review of medical and family history, goals, and expectations.
• Screening for contraindications to HRT.
• Physical exam, including vital signs.
• Education about HRT options, effects, and side effects.
• Shared decision-making to select a regimen.
• Scheduling labs and preparing for initiation of treatment
• Prescribing initial hormones and giving instructions, will be done after labs have been reviewed, this can be done virtually or in person. 
• This visit does not include labs needed for hormone optimization, but will be ordered and you may utilize your insurance if you prefer.
• You’ll have direct access to your provider via phone, text, or email, plus same- or next-day follow-ups/urgent health concerns visits.

** Monthly treatment membership costs range from $70 to $250, depending on plan created**
