This can be a credit card, HSA/FSA card.
This card will be kept on file for future payments.

Please schedule the appropriate intake visit for the package you would like to purchase. 
Once you have booked the intake visit, I will have to manually add the package to your account. 
You will be charged for the entire package at that time.

For All other visits, you will put a card on file but will not be charged until the time of your visit!

Cancellation Policy: Cancellation fee is $100 for a visit cancelled within 24 hours of appointment time!
No Shows will be charged full cost of visit!
