For your first visit, choose either in person or virtual Initial consultation. This is the first of 2 visits included in one price. For your second visit, choose Follow up for initial consult, in person or virtual. All other subsequent visits are a follow up of 30 or 60 minutes.
Next Chapter Hormone is a direct access clinical practice. We do not process health insurance, we are happy to provide you with a superbill that you can submit to your insurance company. We also accept HSA and FSA cards for payment in addition to debit and credit cards. At present time, we are unable to see anyone who is covered under Medicare at all (even when they intend to pay out of pocket). We are hoping this will not be the case very soon, we will update when able.
