About Lifesong Health
Lifesong Health is a small, growing telepsychiatry practice serving adults across Virginia, with limited in-person availability in Forest. We practice whole-person care—evidence-based medication management plus lifestyle, nutrition, and integrative approaches—to help you regain energy, clarity, and balance.

About Your Provider
Fawn Workman, PMHNP-BC, FNP-C, is a board-certified Psychiatric Mental Health and Family Nurse Practitioner with training in psychopharmacology, psychotherapy, metabolic health, and integrative care. She listens first, looks at root causes, and builds a plan that fits your goals—delivered by secure video with timely follow-up.


Payment Options

We are primarily self-pay. A limited panel of active commercial-insurance patients is managed through third-party partners (Alma; Headway for Sentara Health Plan).

• To ask about insurance availability—or to join the insurance waitlist—email [email protected]. As of 8/8/2025, some insurance slots are available for new patients and are filling quickly.
• Otherwise, please book as a new cash-pay patient below.
• Third-party insurance applies to mental-health services only; wellness/metabolic-only care is self-pay.
• We are not accepting Medicare at this time.
• All requested appointments are contingent on practice approval.


How Payment Is Collected

A card on file is required to reserve your time. Nothing is charged at booking; your visit fee is processed during the session (self-pay). Insurance copays/coinsurance are collected by the third party. You must keep an active card on file. A fee may apply to unpaid balances over 30 days.


Appointment Types & Self-Pay Fees

(You are charged for the full time slot you schedule at the time of the appointment.)
Integrative Psychiatric or Wellness Intake — 75 min — $360
Standard Follow-Up — 25 min — $180
Brief Med-Check — 10 min — $150 (by NP approval after at least one follow-up; stability required)
Extended Follow-Up — 50 min — $300 (by approval; please request if desired)
Additional visit types/testing—please ask the office.


Cancellation, Late Arrival & No-Show

Free-cancel window
> 30-minute visits (intakes, extended): 48 hours or more
≤ 30-minute visits (standard follow-up, med-check): 24 hours or more

Fees (charged to card on file)
≤ 30-minute visits: $75 late-cancel / no-show
> 30-minute visits: $175 late-cancel / no-show

Late arrivals are treated as a late cancel and the same fee applies.
Reference cutoffs: 5 min (med-check), 10 min (25–50 min follow-up), 20 min (intake).
If you have trouble joining a telehealth visit, call or text 434-260-3380 before your start time. Insurance does not cover late fees.


Insurance Waitlist & Switching Tracks

You may start as cash-pay and request placement on our insurance waitlist if the panel is full. When an active insurance spot opens, we can move your future visits to third-party billing. To keep billing clean, tracks do not mix and frequent back-and-forth switching isn’t available. (If your insurance ends, the partner changes networks, or our contracts change, we’ll discuss options.)

HOW TO BOOK — ESTABLISHED INSURANCE (THIRD-PARTY  Alma and Headway) PATIENTS

• If you already see us through insurance, choose one of these in the portal:

Follow-Up Telehealth — Sentara (Headway)
• Complete any required forms in Optimantra and in Alma/Headway.
• Need to reschedule? Do it inside your Alma/Headway portal.
• Please follow our late/cancel policy (≤30 min: 24 h; >30 min: 48 h).
• Do not book a cash-pay slot unless you’ve arranged a track change with us.


HOW TO BOOK — ESTABLISHED SELF-PAY PATIENTS

• Complete any forms in Optimantra.
• Need to reschedule? Use your Optimantra portal.
• Please follow our late/cancel policy (≤30 min: 24 h; >30 min: 48 h).


HOW TO BOOK — NEW PATIENTS

Option A: CASH-PAY (fastest)

  1. Select Integrative Intake — 75 min — $360 on this page.

  2. Add a card on file (no charge at booking; fee runs during the session).

  3. Pick your time (telehealth shown here; call for limited in-person in Forest).

  4. Complete Optimantra intake forms within 24 hours. Send us a copy of your drivers license to [email protected] . We review and confirm before accepting the appointment.

Option B: USE INSURANCE (limited panel via third party)

  1. Email [email protected] with: full name, DOB, mobile number; plan name; photos of your insurance card (front/back) and driver’s license/ID; and a brief reason for your visit.

  2. If an insurance spot is open, we’ll send your Alma (or Headway for Sentara) onboarding link.

  3. Complete Optimantra and Alma/Headway intake forms within 24 hours. We review and confirm before accepting the appointment.

  4. If the panel is full, we’ll add you to the insurance waitlist. You may still start now as cash-pay and switch future visits to insurance when a spot opens.

 

 Choose this if you have COMMERCIAL (Not Medicaid or Medicare) Sentara Health Insurance (formerly Optima) for a new client consultation. You will be required to pay your co-pay/ co-insurance at the time of the visit or upon receipt of superbill after insurance determination. 
Only book via this visit type if you have a commercial Sentara Health Insurance (formerly Optima) plan and are an established patient of Lifesong Health. You will be required to pay your co-pay/ co-insurance at the time of the visit or upon receipt of superbill after insurance determination.
