This is do determine if our cash pay membership model is a good fit for you, and if you need labs you need prior to your initial weight loss or hormone replacement visit.
Basic financial questions:
We do not bill your insurance. We are a cash-pay clinic only.
Female hormone replacement therapy is $1500 a year when billed monthly at $125. Otherwise, it is $2000/yr
This plan includes thyroid check, monitoring, and oversight if thyroid replacement is necessary

You will receive a call from our office at your appointment time. Please note this number and be ready for a call (435) 817-9875.
Following a provider consult, we will either provide you with a lab requisition to take to a lab of your choice, or you can upload or email recent relevant labs to your Core Health provider for a lab review. When we provide you with a lab requisition, you may take it to a lab of your choice. Again, we do not bill insurance or guarantee insurance coverage for your labs. We encourage you to provide the lab of your choice with your insurance information, to have your labs run through insurance billing to have the greatest likelihood of having your insurance provider cover at least a portion of your laboratory work.
Please aim for an AM (8AM - 9:30AM), fasting lab draw. Go well hydrated (beginning the day before) with water only.
