Book our Evaluation Visit with your dedicated Nurse Practitioner. At this visit, we will order and review labs and provide a full PCOS assessment, diagnosis, and initiate a plan of care including medications. If you have existing labs, please make sure to upload under documents on your portal, otherwise we will order new labs and a metabolic work up.
*Costs of labs, imaging, and prescriptions are not included. However, prices are discounted for all states with the exception of New York and often lower cost than using insurance. *Per New York State Laws: Discount labs not available. However, you may use your insurance to cover labs. Duration: 30 minute video visit

*Costs of labs, imaging, and prescriptions are not included. However, prices are discounted for all states with the exception of New York and often lower cost than using insurance. *Per New York State Laws: New York patient subscription includes one monthly telehealth visit, all additional telehealth visits are $35 per visit. Discount labs not available. However, you may use your insurance to cover labs. *For the pre-paid 3, 6, 12 month membership plans, there are no refunds as you are receiving discounts for these packages if you commit to an extended program. *Termination: Both the patient and provider may terminate the membership agreement effective immediately. PREPAID PLANS DO NOT AUTO-RENEW. You will need to purchase another pre paid plan if continued treatment is desired. If membership is cancelled or you do not renew your prepaid membership plan, there will be a $50 re-enrollment charge if you return in the future. You also lose grandfathered price privilege. There is a 7 day grace period after notification of subscription or prepaid plan expiration, in which you must respond to avoid the $50 re-enrollment charge upon reinstatement.
