**Please Note: Intake and Consent Forms sent by email are required to be submitted electronically 48 hours before the day of your appointment so that Dr Reap has enough time to prepare for your appointment. Your appointment may be cancelled and rescheduled if forms are not submitted on time. 

Only health insurance companies in-network/accepted are Mountain Health Co-Op, BlueCross BlueShield, Allegiance, Cigna, Pacific Source, First Choice Health, and Aetna. For all others, the clinic is out-of-network but can provide a superbill to submit.
*Unfortunately, Medicare, Tri-Care/Veteran, and MT state Medicaid does not cover naturopathic care at this time. There are some Medicare supplemental insurances that do cover, so please contact insurance company to check coverage. Efforts are in progress to expand naturopathic care coverage.
Sports Physical Exam 30 Min
The sports physical is also known as a pre-participation physical evaluation (PPE). The sports history and exam helps determine if it's safe for an individual to participate in sport(s). Most states require students have a sports physical before they can play school sports. 
60-minute HBOT Session 75 Min
Schedule your 60-minute hyperbaric oxygen therapy session. 
**Note: If you are new to hyperbaric oxygen therapy at the clinic, you need to schedule an initial consultation to determine if you are approved for HBOT. 

Hyperbaric Oxygen Therapy (HBOT) provides the body with more oxygen at a higher pressure to help assist the body's healing process from injury or illness. 

Benefits Include: Stimulating new tissue growth, Increase circulation by creating new blood vessels, Increase regenerative/anti-aging processes, Assist body in fighting infections, reduce swelling and inflammation, and Improve outcomes in neurological conditions.

90-minute HBOT Session 110 Min

HBOT Consultation 15 Min
Consult with Dr. Sam to discuss how hyperbaric oxygen therapy may benefit you. It's best to come in person to see the chamber, learn how it works, and explore how this innovative therapy can support your health goals. Together, you can review your medical history and determine if this therapy is the right fit for your wellness journey.

DOT Physical 30 Min
A Department of Transportation (DOT) physical is a physical exam that is required of people who drive commercial vehicles. The point of a DOT physical is to make sure that people who are driving commercial motor vehicles are physically, mentally, and emotionally fit. 
*Please provide Drivers License/State ID in personal ID section of online scheduling. 

Non-DOT / Work Physical 30 Min
A non-DOT / work physical is a type of medical examination that is not mandated by the federal government but is optional and may be required by some employers. Unlike the DOT physical, it can vary based on the employer’s requirements, the position’s physical demands, or the industry’s standard. Non-DOT physicals are used to assess the overall health of the employee, identify potential health issues that can impact job performance, and minimize workplace accidents. These physicals vary from employer to employer, and the employer can customize the physical to meet their needs.
