New Patient Intake (For patients not seen @ Wonderland, or by provider, in the past 3 years). 

Medical Decision Making (MDM): High complexity:
  • Assessment of conditions posing an immediate threat to life or bodily function (e.g., active suicidal ideation with a plan).
  • Initiation or management of high-risk medication like Clozapine or Lithium.
  • Making the decision for hospitalization or intensive outpatient treatment.
  • Extensive data review and analysis required for complex differential diagnosis.

Time Based Billing 
  • 60-74 minutes
  • face to face time (excluding psychotherapy time)
  • non face to face time (Documentation, care coordination, records review) on day of visit
Services for office visits (E/M codes 99202–99205 and 99212–99215) are billed based on one of two criteria:
  • Medical Decision Making (MDM): The complexity of your health needs, data reviewed, and risks involved.
  • Total Time: The total time the provider spends on your care the day of the visit, including documentation and face-to-face time.
The provider selects the method that best represents the work performed during your visit.

  • One stable chronic illness OR acute, uncomplicated illness.
  • Limited data review/ordering of labs or reviewing prior simple test results.
  • Low risk management decisions (e.g., continuing current medication, lifestyle adjustments).
Time Based Billing
  • 20-29 minutes
  • face to face time (excluding psychotherapy time)
  • non face to face time (Documentation, care coordination, records review) on day of visit

Medical Decision Making (MDM): Moderate complexity
  • One or more chronic illnesses with exacerbation/progression OR two or more stable chronic illnesses.
  • One undiagnosed new problem with uncertain prognosis OR an acute illness with systemic symptoms.
  • Review/order of pertinent diagnostic tests (labs, imaging) or review of external medical records.
  • Interpretation of tests or discussion of management with an external physician.
  • Prescription drug management is required.
Time Based Billing
  • 40-54 minutes
  • face to face time (excluding psychotherapy time)
  • non face to face time (Documentation, care coordination, records review) on day of visit
