By clicking "proceed", you agree that at the time of your visit you will be:
1) Physically located in the state of Alabama
2) Between the ages of 19 and 64
2) NOT be covered by a Medicaid or Medicare plan
*A $35 deposit is required at booking, which will be credited towards your total account balance. Cancellations or no-shows within 24 hours of the scheduled appointment will be subject to a cancellation fee of $35.
As a new patient, we warmly welcome you to take advantage of our comprehensive package which includes TWO visits that will set you on the path to optimal health.
During your first visit, we'll conduct a thorough initial evaluation where we'll discuss your health history, concerns, and goals. This will allow us to gain a comprehensive understanding of your unique needs and create a tailored plan that's just right for you.
Following your initial evaluation, we'll schedule your second visit (free of charge), during which we'll review labs and work with you to develop a customized care plan that will help you achieve your health and wellness goals. With our comprehensive package, you can rest assured that you'll receive the highest level of care and attention, designed specifically for you.
The second visit is included in your New Patient Package. Only book this if you have completed your first visit. At this visit, we'll discuss your results in detail and develop your customized care plan that includes recommendations for lifestyle changes, medication, and other treatments as needed.
This visit includes any of the following: lab review, medication refills, consultation for new medical concerns
