Patient will answer in-depth intake.  Doctor will review intake prior to appointment, using appointment time for questions, answers, physical exams, and recommendations for testing and treatment plan.

Treatment plans are based on patient preference and doctor's recommendations.

Consultation and exam fee does not include labs.  Doctor will advise patient of recommended labs and their cost during the appt.
