About Lifesong Health

Lifesong Health is a small, growing telepsychiatry, weight, and wellness practice serving teens and adults across Virginia with intentional care. We offer limited in-person availability in Forest. We practice whole-person care—evidence-based medication management plus lifestyle, nutrition, and integrative approaches—to help you regain energy, clarity, and balance. 

About Your Provider

Fawn Workman, PMHNP-BC, FNP-C, is a board-certified Psychiatric Mental Health and Family Nurse Practitioner with training in psychopharmacology, psychotherapy, metabolic health, and integrative care. She listens first, looks at root causes, and builds a plan that fits your goals—delivered by secure video with timely follow-up.


Payment Options

  • Self-Pay: Available for all new patients.
  • Sentara Health Plan (Commercial): In-network.
  • Alma / Headway: Limited to existing patients (not accepting new patients).
  • Medicare/ Medicaid: Not accepted. You may elect to self pay with a private contract agreement, understanding your insurance will not reimburse you. 
  • All appointment requests and prospective clients are reviewed and contingent on practice approval.

How Payment Is Collected

  • A card on file is required to reserve your time.
  • Nothing is charged at booking.
  • Self-pay fees are processed the day of service (or shortly after).
  • Sentara copays/coinsurance are collected at your visit.
  • Alma/Headway patients pay through those platforms.

Self-Pay Fees

  • New Patient Intake (75 min): $360
  • Standard Follow-Up (25 min): $180

Other services may be available—please ask the office.

If a visit runs over, the fee increases to the next level.

New Patients

  • Self-Pay Prospective Patients: Self pay new clients (all new clients except those with Sentara Health Plan commercial plan): Select Self Pay- New Patient- Telehealth 
  • Sentara Health Plan Prospective Patients: Select Sentara Health Plan- New patient- Telehealth

Next Steps:

  1. Add a card on file (no charge until the day of session).
  2. Choose your appointment time (telehealth; limited in-person available in Forest—contact us if needed).
  3. Complete intake forms in the Optimantra Patient Portal within 24 hours.
  4. Email [email protected] with:
    • A copy of your driver’s license
    • A copy of your insurance card (front/back—even if self-pay, in case labs or medication authorizations are needed)
  5. Your appointment request will be reviewed and confirmed.

Established Patients

  • Self-Pay: Select Self Pay- Follow Up- Telehealth- 25 minutes (standard)
    • alternate options by approval only and scheduled internally.
  • Sentara Health Plan (Commercial): Select Sentara Health Plan- Follow Up- Telehealth
  • Alma / Headway: Select Alma*/ Headway*- Follow Up- Telehealth
  • Hello Hope Legacy Patients- choose this option for follow up appointments. Ensure your membership is paid up to date.

Cancellation, Late Arrival & No-Show Policy

  • Cancel Window:
    • Intakes/extended visits (≥30 min)/ in person visits: cancel ≥48 hrs in advance
    • Standard telehealth visits: cancel ≥24 hrs in advance
    • From the patient portal you can cancel 2 days in advance; if it's less than 2 days from your appointment, please Spruce message, text or email the office (fee may apply). 
  • Fees:
    • $75 for late cancel/no-show of visits 30 min or less
    • $175 for late cancel/no-show of visits 30 min or longer
  • Late arrivals (past 10 minutes for most visits) are treated as late cancels, will be charged a fee and required to rebook
  • If you have trouble joining telehealth, call or text (434) 260-3380 before your "late" time.
