For DIRECT PRIMARY CARE MEMBERS ONLY. Telehealth visit
An Initial Weight Loss Consultation is conducted via telehealth and serves as the first step in creating a safe, personalized treatment plan.  During this visit, we’ll review your full medical history, current medications, and past weight loss experiences. We’ll discuss your weight history, lifestyle factors, and any underlying health conditions that may impact your ability to lose weight. Together, we’ll talk through your goals and determine whether medical weight loss is appropriate for you.

If appropriate, we will review medication options, including how they work, expected benefits, potential side effects, and what monitoring is needed. This visit is focused on building an individualized plan that supports sustainable, healthy weight loss — not a one-size-fits-all approach.

Because this visit is done via telehealth, some patients may require additional lab work or in-person follow-up to ensure safety and effectiveness before starting treatment.
A Weight Loss Follow-Up Visit is conducted via telehealth to monitor your progress and ensure your treatment plan is working safely and effectively.

During this visit, we’ll discuss how you are tolerating your medication, review any side effects, and determine if dose adjustments are needed. We’ll also talk through your progress, challenges, and successes with nutrition and physical activity.

This is a time to reassess your goals, review diet and exercise habits, and make personalized adjustments to support continued, sustainable weight loss. Ongoing follow-up helps ensure your plan remains effective, safe, and tailored to your needs.
A telehealth Visit for NON-MEMBERS offers convenient, same-day access to a medical provider without needing a membership. These virtual visits are ideal for addressing many common, straightforward health concerns from the comfort of your home.
Telehealth can often be used to evaluate and treat conditions such as:
  • Cold, flu, or COVID symptoms
  • Sinus infections
  • Sore throat
  • Allergies
  • Pink eye
  • Rashes or minor skin concerns
  • Urinary tract infections (UTIs)
  • Minor gastrointestinal issues (nausea, diarrhea, reflux)
  • Medication questions or short-term refills
  • Mild anxiety or sleep concerns
However, not all medical issues are appropriate for telehealth. Some conditions require a hands-on physical exam, in-office testing, or urgent/emergency care. Chest pain, difficulty breathing, significant injuries, severe abdominal pain, neurological symptoms, or complex/chronic concerns may need in-person evaluation.
An in-Person Visit for non-members provides convenient access to high-quality primary care without the need for a membership.
These visits are ideal for evaluating and treating a wide range of common concerns, such as:
  • Cold, flu, or respiratory symptoms
  • Ear infections
  • Sore throat or strep concerns
  • Sinus infections
  • Urinary tract infections (UTIs)
  • Minor injuries (sprains, strains)
  • Rashes or skin conditions
  • Abdominal discomfort
  • Blood pressure concerns
  • Minor procedures (when appropriate)
If needed, additional in-office testing such as labs, swabs, or other point-of-care tests may be recommended. Additional testing fees may apply, but we use transparent pricing — meaning all costs are discussed with you ahead of time so there are no surprises before any treatment or testing is performed.
An in-Person Visit for non-members with chronic conditions offers personalized evaluation and management without the need for a membership.
These visits are appropriate for patients needing support with ongoing health concerns such as:
  • High blood pressure
  • Diabetes
  • Atrial fibrillation
  • Thyroid disorders
  • High cholesterol
  • Asthma or COPD
  • Other stable chronic conditions
During your visit, we will review your medical history, current medications, symptoms, and overall disease control. We’ll discuss treatment options, medication adjustments if appropriate, and strategies to help better manage your condition.
If needed, additional in-office testing such as labs, EKGs, or monitoring may be recommended. Additional testing fees may apply, but we use transparent pricing — meaning all costs are discussed with you ahead of time so you can make informed decisions before any testing or treatment is performed.
An in-person visit for non-members seeking a CDL Exam provides a convenient, straightforward option for completing your required Department of Transportation (DOT) physical.

During this visit, we will perform the full CDL/DOT exam, including review of your medical history, vital signs, vision screening, and physical assessment to ensure you meet federal safety standards for commercial driving. A standard DOT urine screening is included in the exam and pricing.

Please note: this visit includes the required medical urine screening only. Drug testing is not performed.

Any required documentation will be completed at the time of your visit. If additional evaluation is needed based on DOT guidelines, this will be discussed with you beforehand. Our goal is to provide an efficient, transparent process to help you stay compliant and on the road safely.
