This is for an initial intake to be a weight loss patient at Fair Winds Direct Care with Morgan Massey, APRN. We will schedule a 1 hour appointment to discuss your history, take your measurements, draw a baseline set of labs, and finalize a personalized weight loss plan for you.
Forms will be emailed to you to complete prior to your appointment. If not completed, you may be asked to reschedule.
A $50, non-fundable, fee will be charged to book your appointment. This will be credited towards the visit fee of $285
