Comprehensive Intake Package 

Step 1: Schedule you appointment and once it is confirmed you will receive an email with consents, intake forms that will need to be filled out prior to your visit and information on registering for our portal. If you do not receive this information, please email us at [email protected]. 

Step 2: We will contact you to schedule your lab draw. 

Step 3: Once your lab results are back we will complete your comprehensive health evaluation. Please schedule this knowing it may take 7-10 days to get your lab results back. Ex. if there is an appointment available in the next few days, we will not have time to get your labs back, so please schedule about 10 days from today

The customized lab panel includes:

• Metabolic Health: Fasting glucose, insulin, HbA1c
• Hormone Optimization: Thyroid panel (including antibodies), sex hormones, cortisol
• Nutrient Status: Vitamin D, B12, and iron panel
• Inflammation & Wellness Markers: CRP, homocysteine, ferritin

A detailed review of your medical history, symptom assessment from a functional perspective, evaluation of lifestyle, nutrition and stress-level along with your lab results and in office exam will help us develop a plan to achieve your personal health goals and priorities. 

You will leave this visit with:

• A clear and thorough summary of your lab findings
• Customized recommendations for hormone balance, metabolic support, lifestyle shifts, and supplement guidance
• Prioritized treatment pathway with suggested timelines, retesting intervals, and targeted therapies.

We do offer a 10% discount for first responders (police, fire, medic), nurses and teachers. You will need to show you ID prior to receiving the discount. 

If you are currently taking a GLP medication for weight loss and would like to transfer to our clinic for management please schedule here to establish care @ Innovate Wellness Carolina. This visit which is $199 includes an initial consult with the provider and lab work.

The cost going forward is based on which medication you are currently taking and the dosage. We bill for 2 months at a time.
