This is a monthly visit to discuss weight loss progress, treatment and goals. 

This is your initial weight loss consultation.  We will review you health history, current lifestyle, goals, and weight loss options. 

Enjoy a thorough, personalized, in-person visit with a provider to discuss optimal hormone health. This visit is for new patients who have not been on hormone replacement therapy with our practice. The visit includes initial consultation, lab order/review and 90 day prescription if warranted.  Package does not include the cost of labs or medication.

Telehealth and in person appointments available.  

This visit will be TELEHEALTH and for patients who can not come into the office at this time. This visit is for patients who have already established as an HRT patient and just need a follow up visit.
