A comprehensive sports physical required for school athletics participation. This visit includes a full review of medical history, vitals, physical exam, and completion of the required clearance form (FHSAA or school-specific form if provided). This exam is designed to ensure your teen is healthy and safe to participate in physical activity. If your child has a chronic condition or complex medical history, additional follow-up may be recommended.
