During this appointment, we will:
  • Conduct a full psychiatric and substance use assessment, including medical and psychosocial history.
  • Evaluate your eligibility for Vivitrol (naltrexone), Sublocade (buprenorphine extended-release), or Brixadi (buprenorphine extended-release), depending on your clinical needs and treatment goals.
  • Discuss the mechanism, benefits, side effects, and safety considerations of each medication option.
  • Review past and current medication and treatment history, including prior detox or withdrawal experiences.
  • Assess for co-occurring mental health conditions such as depression, anxiety, PTSD, or bipolar disorder.

Description:This follow-up visit is conducted 48 to 72 hours after a PPD (Mantoux) skin test is administered to screen for tuberculosis (TB) exposure. During the appointment, a licensed provider will examine the injection site on the forearm to assess for any induration (raised, hardened area), measure the reaction in millimeters, and determine if the result is positive or negative based on CDC guidelines and the patient’s risk factors.
What to Expect:
  • Visual and tactile examination of the test site
  • Accurate measurement and documentation of any reaction
  • Explanation of results and next steps (if any)
  • Referrals for further evaluation (e.g., chest X-ray, IGRA) if the result is positive

Physician Statement of Good Health & PPD Reading Process
Description:
You are receiving a Physician Statement of Good Health and undergoing a Tuberculosis (TB) screening using the PPD (Purified Protein Derivative) skin test. The PPD test helps determine whether you’ve been exposed to tuberculosis bacteria. A follow-up visit is required 48 to 72 hours after administration to check for a skin reaction.
 Process:
1. Initial Visit (Today)
  • You were seen for a Work Physical Examination, including a PPD test placed on your forearm.
  • You have no current health issues that would prevent you from performing work duties.
  • A preliminary Statement of Good Health has been prepared, pending PPD results.
2. Return for PPD Reading (48–72 Hours After Placement)
  • You must return between 48 and 72 hours from the time the test was placed (e.g., if placed Monday at 3 PM, return between Wednesday 3 PM and Thursday 3 PM).
  • At the follow-up:
    • A licensed healthcare provider will measure the induration (raised, hard area) on your arm (not redness).
    • Results will be documented as negative or positive:
      • Negative: No or small induration (typically <10mm for low-risk individuals).
      • Positive: Larger induration (≥10mm or ≥5mm depending on risk factors) → may require chest X-ray or additional testing.
3. Receiving Statement of Good Health
  • If the PPD result is negative, the final Physician Statement of Good Health will be signed and issued.
  • If positive, further evaluation may be needed before clearance can be given.
Important Notes:
  • Do not cover the test site with a bandage or apply lotion.
  • Monitor for itching, swelling, or discomfort, but this is not unusual.
  • You must return for the reading even if you see no reaction.
