House Call Services for Individuals with Disabilities & Caregivers
We are proud to offer in-home (house call) services for children and adults with disabilities, as well as their caregivers. These visits are designed to improve access to care for individuals who may have difficulty leaving their home, helping to reduce gaps in care and maintain ongoing connection with a healthcare provider.
What to Expect:House call visits are focused on patient concerns and chronic condition management. They are structured to be time-efficient in order to accommodate all patients requesting services, while also maintaining provider scheduling, safety, and accountability across visit locations.
These visits may be used for sick visits, wellness visits, general health and wellness checkups, or management of routine and chronic conditions. Patients can expect their visit to last no more than 30 minutes.
There are both benefits and limitations to receiving care in the home compared to a traditional clinical setting. Certain evaluations, diagnostics, or treatments may require follow-up in a clinic or specialty setting.
Scheduling, Payment & Billing:

• Cash-pay visits must be paid in full at the time of booking.
• Insurance-based visits will be billed to your plan. Applicable copays, deductibles, or non-covered services may apply.
• A travel fee may be assessed depending on your insurance plan and/or service location.

Safety & Home Requirements:To maintain a safe and respectful environment for all parties:

• All animals must be secured prior to the visit.
• Any weapons must be safely locked and put away.
• No nicotine or vaping use is permitted during the visit.
• Families are required to disclose any known infestations (including insects or rodents) prior to scheduling.

Care Team & Visit Support:At times, additional staff may accompany the healthcare provider to assist with documentation, supplies, or patient care support.
