Your first visit is a thorough and personalized consultation. We review your health history, symptoms, and relevant lab data to gain a complete understanding of your health before recommending any next steps. Treatment is never one-size-fits-all. Every decision is made collaboratively and based on a proper clinical assessment and your goals.

Fee & Payment
A consultation fee is charged at the time of your appointment. Rejuvenate Health Clinic is a cash-pay practice. We do not bill insurance.

A card is taken at the time of booking and is kept on file.

Please read before booking:
Cancellations or rescheduling must be made at least 24 hours before your appointment.
Appointments cancelled with less than a 24-hour notice will result in a $50 charge to your credit card on file.
No shows will also incur the $50 fee.

This is for a TELEHEALTH visit from the comfort of your own home.
You will get an email to access your patient portal to fill out forms at least 24 hours prior to your appointment. There will also be a link emailed to you when you schedule your appointment to access the telehealth visit at the time of your appointment.
