During this visit, we may:

• Review progress, symptoms, and side effects
• Review medications and make adjustments if appropriate
• Discuss lab results or determine if additional labs are needed
• Update your treatment plan based on your goals and response to therapy

Visit length: 15–20 minutes
Pricing: $75 for the visit This fee covers the provider visit and care planning.
Additional costs: Labs, medications, or treatment packages are not included and may result in additional charges depending on medical necessity and the treatment plan selected.

• You must be available for real?time audio and video
• This visit is required periodically to continue medication management
• Medication adjustments are not guaranteed and depend on clinical appropriateness

This is an asynchronous follow up visit completed through a secure online questionnaire and provider review. A live video visit is not included.
This visit is appropriate if you:

• Are doing well on your current weight loss treatment
• Need a routine medication refill
• Have no new or concerning symptoms
• Do not require a medication change or detailed discussion

The provider will review your submitted information and respond with next steps through the patient portal.

Pricing: $55 for this visit This fee covers provider review and care planning.
Additional costs: Lab work, medications, or treatment packages are not included and may result in additional charges depending on medical necessity, treatment plan selected, or insurance coverage.
