99204 – Comprehensive Psychiatric Evaluation (45–59 minutes)
In-person or Telehealth | Children, Adolescents, & Adults
This service is a comprehensive initial psychiatric evaluation for new patients. It includes a detailed assessment of current symptoms, psychiatric history, medical history, family and social history, trauma exposure, substance use, and functional impairments. A full diagnostic formulation is completed using DSM-5-TR criteria.
For children and adolescents, collateral information from parents, caregivers, or schools may be incorporated as clinically appropriate.
The visit concludes with:
  • Diagnostic impressions
  • Initial treatment recommendations
  • Discussion of therapy modalities and/or medication options
  • Education regarding risks, benefits, and alternatives
This level is typically used for moderate-complexity presentations.
New Patient Psychiatric Evaluations
99205 – Comprehensive Psychiatric Evaluation (60–74 minutes)
In-person or Telehealth | Children, Adolescents, & Adults
This is an extended, high-complexity initial psychiatric evaluation for patients with complex or severe presentations. In addition to all elements of a standard psychiatric intake, this service allows time for in-depth assessment of:
  • Multiple psychiatric diagnoses
  • Significant trauma histories
  • Complex psychosocial stressors
  • Medical or neurological comorbidities
  • Safety concerns or functional impairment
For pediatric patients, extensive caregiver involvement and review of external records may be included.
This service includes:
  • Comprehensive diagnostic clarification
  • Development of an individualized, multi-layered treatment plan
  • Medication initiation or adjustment when indicated
  • Coordination considerations with other providers
