This is a telehealth video visit
This includes initial assessment (Telehealth visit 30- 60 minutes ). 
Includes:

• Initial assessment (Telehealth video visit approx 45-60 minutes)
• First month’s prescription (not medication cost) sent to pharmacy
  • Pay out of pocket medications options are also available.
• Prior authorization if needed
• Guidance and  education.
• Lab work  can be ordered through your insurance.

This is a medical appointment. 
You must be present in Massachusetts during the visit, where I am licensed.
 After Initial consult you will be required to check in with provider approximately every 4 weeks through the portal to decide your dosing schedule and Monthly Subscription fee is required for ongoing care.

Please note: Most insurance companies REQUIRE that you have participated in a comprehensive weight management program that encourages behavioral modification, reduced calorie diet, and increased physical activity with continuing follow-up for at least 6 months prior to using drug therapy = please  remember this as you are filling in your questionnaire 

You will not be refunded for “no shows” or cancelations less than 24 hours in advance.
