This is a Return/Follow-Up Visit via telehealth. You must be within the state of Ohio, New York or Colorado to conduct this visit. This visit is reserved for established patients who are currently enrolled in an Empowered Wellness membership and have been seen within the past 12 months as part of ongoing membership care. New patients should select a New Patient Visit.

This is a VIRTUAL appointment with Courtney Shihabuddin, NP. You will receive a link to join your visit a few minutes before it's set to start. 

Your journey begins with a comprehensive 60-minute consultation. During this visit, Dr. Shihabuddin will:

• Review your detailed medical history and health intake form 
• Explore your symptoms and goals collaboratively
• Discuss potential root causes and treatment options
• Review your lab results and ensure understanding
• Formulate a treatment plan utilizing shared decision-making and initiate treatment, if appropriate

This visit sets the foundation for a truly individualized plan of care.

RECLAIM Health Coaching


RECLAIM is our 12-week nutrition and health coaching program designed to help you build sustainable habits, improve energy, and feel more confident in your wellness routine. With monthly visits with our registered dietitian, weekly coaching emails, and supportive handouts, you’ll gain the knowledge and structure to care for your health in a realistic, sustainable way.

RECLAIM is led exclusively by our Registered Dietitian and is designed to support women through personalized nutrition, lifestyle, and health coaching. This program does not include nurse practitioner visits, medical care, or prescription management.

The RESTORE Membership is designed for women who are starting hormone therapy or making active adjustments to improve how they feel. This tier includes more frequent visits and close collaboration with your provider, offering the steady, supportive care that’s especially important in the early phases of treatment.
RESTORE is ideal for your first year of hormone therapy or when you’re navigating persistent symptoms of perimenopause or menopause. It also provides comprehensive support for midlife concerns like thyroid dysfunction, PCOS, PMDD, hair changes, gynecologic issues, sexual wellness, and bone density concerns such as osteopenia or osteoporosis.
