IMPORTANT: Good Faith Exam Requirement (read before booking)
Before your appointment, all patients are required to complete a yearly Good Faith Exam with one of our virtual providers to help ensure treatment safety.
This exam can be completed any time before your scheduled appointment using the following link: Doctovia Good Faith Exam.
If you have already completed a Good Faith Exam within the last 12 months and have had no changes to your health history since your last visit, you may skip this step after booking. The Good Faith Exam includes an additional $27 fee added to your service. If the exam is not completed before your appointment, please arrive 10 minutes early as additional time may be needed to complete it onsite.
Aesthetic Service Exception
A Good Faith Exam is NOT required for the following aesthetic-only services:
• Waxing
• Facials
• Dermaplaning
• Nano Infusions
Please note: Microneedling DOES require a Good Faith Exam.
If you are unsure whether your scheduled service requires a Good Faith Exam or are unsure when your last exam was completed, please contact us before completing the exam to avoid unnecessary charges.
If you prefer to schedule your Good Faith Exam for a later date or time before your appointment, the link to join your virtual appointment will be sent to the email address used during scheduling.
Please note: if you complete the Good Faith Exam and later cancel or no-show your appointment, the $27 fee will still be charged to the card used at booking; however, your exam will remain valid for one year should you reschedule.
What is your cancellation policy?
We understand that unforeseen life events happen! Please let us know if you need to cancel your appointment and we will work to get you rescheduled if desired. However, we do have a 24-hour cancellation policy. If you cancel at least 24 hours prior to your booked service, you will not be charged. If you cancel in less than 24 hours of your appointment, or do a no-show, you will be charged 25% of your booking fee.
