This is a flat rate for basic service, no insurance, cost per visit.  

This 30 minute appointment is a follow up to review patient status with a certified functional medicine nurse practitioner.  This is for an existing patient only.  It will include a brief discussion of next step options.  

This appointment is required before a refill is authorized. It includes your monthly fee and medication. 

This is an initial screening for weight loss options. It can include GLP-1 or holistic options.  This does not include your first month’s fees and medication.
