This is a flat rate for basic service, no insurance, cost per visit.  

This is a 30 minute follow up appointment to discuss how you are doing with your treatment plan and to make any changes if needed.  

This 60 minute visit includes a tailored treatment plan based on personal lab results and clinical judgement in collaboration with the patient.  It includes suggested dietary changes, lifestyle recommendations, supplement and/or medications, and Q&A. This is for an established patient who has already completed their initial consultation and their Lab Review appointments. 

This 60-90 minute consultation includes a comprehensive history and evaluation of your medical health and lifestyle with a certified Functional Medicine Nurse Practitioner.  It will include discussion of lab testing recommendations.  Labs will be purchased separately.  

Next steps include ordering labs and appointment to review results when they are available.

Next step options, such as nutrition plan and supplement and/or medication recommendations will be discussed at the Treatment Plan Appointment.  

This is an initial screening for weight loss options. It can include GLP-1 or holistic options.  This does not include your first month’s fees and medication. 

This appointment is required before a refill is authorized. It includes your monthly fee and medication.
