Please note that Opal Integrative Health is a self-pay practice. Lab testing and medications are not included unless specifically noted with your selected service.

Cancellation Policy:
We kindly ask for at least 24 hours’ notice for any appointment cancellations or rescheduling. Appointments canceled within 24 hours of the scheduled time, or no-show appointments, will be subject to a $100 cancellation fee.

This follow-up visit is for established women’s health patients currently receiving care through our clinic. During this appointment, we will review symptoms, treatment response, medications, and laboratory results as applicable. Adjustments to your personalized care plan may be made to optimize hormone balance, symptom relief, and overall wellness.

This appointment is for established patients only. Medication refills and treatment changes are determined at the provider’s discretion.

Women's Comprehensive Care Follow-Up (Telehealth) 20 Min $ 0.00

Women's Comprehensive Care Initial Lab Draw 15 Min $ 0.00

This visit is for the initial lab draw for women’s health patients planning on receiving care through our clinic. During this appointment, we will draw your initial lab panel for review at your intake appointment.

Please arrive fasting for your lab appointment. We recommend no food for 8–12 hours prior to your visit. Water is encouraged, and you may continue taking prescribed medications unless otherwise directed by your provider. Please schedule your appointment accordingly.

Please Note: Initial lab costs typically range from $150–175 depending on personal health history and the labs required. Payment is due at the time of service.

Treatment is not guaranteed. A plan of care will be determined at your comprehensive intake evaluation.
