This 75-minute comprehensive visit is designed for women seeking personalized hormone and preventative care during perimenopause and midlife.

During this consultation, we will:
• Review your medical history and current symptoms
• Discuss sleep, mood, metabolic, and hormonal changes
• Review prior laboratory results (if available)
• Identify preventative care needs
• Develop an individualized plan of care

If appropriate, laboratory testing may be recommended to further guide treatment decisions.
This visit is focused on thoughtful evaluation and structured planning, not rushed prescriptions.

Payment is due at the time of scheduling. A valid card is required to hold on file.

• Opal Integrative Health operates as a self-pay practice. Insurance is not billed for this visit.
• The consultation fee covers the comprehensive evaluation and personalized care plan.
• Laboratory testing, medications, imaging, and supplements are billed separately.
• Lab orders and prescriptions may be submitted to your insurance and/or pharmacy; coverage is determined by your individual plan.
• Hormone therapy or medications are prescribed only when clinically appropriate and are not guaranteed at the initial visit.

Cancellation Policy
We require at least 24 hours’ notice for cancellations or rescheduling. Appointments canceled with less than 24 hours’ notice or missed appointments will incur a $50 fee.
