Follow-Up Visit (Established Patients Only) $ 0.00

Duration: 30 minutes | Format: In-office or Telehealth
This visit is for established patients following up on an ongoing treatment plan—such as hormone therapy adjustments, weight loss progress, or primary care concerns.
We’ll review your recent results, symptoms, and progress, make any medication or dose adjustments, and continue to refine your plan.

Initial Consultation (New Patients)- Primary Care, Hormone Therapy, or Weight Loss $ 150.00

Duration: 60-90 minutes | Format: In-office or Telehealth
This comprehensive first visit is for new patients interested in establishing care for primary care, hormone replacement therapy (HRT), or medical weight loss management.
During this visit, we’ll review your medical history, symptoms, lifestyle, and goals in detail. Your provider will discuss personalized treatment options, order any necessary labs, and outline your care plan moving forward.

DOT Physical (Department of Transportation) $ 100.00

Duration: 60 minutes | Format: In-office only. A comprehensive physical exam required for commercial driver certification. Includes vision, hearing, blood pressure check, urinalysis, and all DOT-required forms. Please bring your driver’s license, eyewear (if applicable), and any medical clearance documentation for chronic conditions.

Sports/School physical $ 50.00

Duration: 20–30 minutes | Format: In-office only
A standard physical exam required for school or organized sports participation. Includes vitals, physical assessment, and completion of school/sports forms. Please bring your form to the appointment.
