Welcome to Simply Womens Care! We are am honored to be able to care for you in your healthcare journey. All visits are cash pay. We accept a variety of payment options including HSA/FSA, credit card, exact cash or check. Most insurance companies will allow you to submit paperwork, which we will provide, for up to 70% out of network reimbursement. We do not guarantee the reimbursement, but will do anything to help you obtain it. Please contact us by email or phone with any questions. Look forward to seeing you soon! 
Basic Annual Exam:
This visit includes a routine wellness check with a breast and pelvic exam, Pap smear (if due), and preventive health screenings. This appointment is for patients without any new or ongoing health concerns. If you have specific problems to discuss, please schedule a separate acute problem visit or comprehensive problem visit. 
Annual Exam + Initial Weight Loss Consultation:
This visit includes a routine wellness exam with preventive screening (breast and pelvic exam, Pap smear if due) along with a personalized consultation to discuss weight loss goals, lifestyle habits, and medical treatment options. Ideal for patients seeking both preventive care and support with sustainable weight management.
Annual Exam + Initial Hormone Consultation:
This visit includes a routine wellness exam (breast and pelvic exam, Pap smear if due) plus a detailed consultation to address symptoms of hormonal imbalance such as fatigue, mood changes, low libido, or hot flashes. We will discuss lab testing and personalized treatment options, including bioidentical hormone therapy if appropriate.
Initial Weight Loss Consultation (In Person):
This visit focuses on your weight loss goals, lifestyle habits, and overall health. We will review your medical history, discuss contributing factors, and explore personalized treatment options—including nutrition, activity, medications, and lab work if appropriate.

Initial Hormone Consultation (In Person): 
This visit is for patients experiencing symptoms of hormonal imbalance such as fatigue, low libido, mood changes, hot flashes, or irregular periods. We’ll review your medical history, discuss your symptoms in detail, and explore appropriate lab testing and treatment options, including bioidentical hormone therapy if indicated.

Acute Problem Visit:
This appointment is for evaluation of a new or urgent issue such as urinary tract infection symptoms, breast lump, vaginitis symptoms, STD testing, or other sudden symptoms. It focuses on one main concern. If you have multiple or complex issues, a longer visit is needed, and we ask that you book the next level appointment (Problem Visit - Comprehensive). 
Problem Visit – Comprehensive:
This visit is for evaluation and discussion of one or more ongoing, complex, or non-urgent health concerns that require more time and attention. Examples include chronic pelvic pain, chronic sexual pain, abnormal bleeding, or multiple symptoms. This extended visit allows for in-depth assessment, discussion of testing, and treatment planning.  
Contraceptive Counseling Visit: 
This visit is for discussing birth control options based on your lifestyle, health history, and reproductive goals. We’ll review hormonal and non-hormonal methods, including pills, patches, IUDs, implants, and more, to help you choose the best option for you. Placement procedures (like IUD or implant insertion) will require a separate visit.  If you are due for your annual exam, we are happy to address contraception options at your annual exam visit.  This appointment is for patients that are up to date on their annual exam or prefer to not have an annual exam at the moment and are looking to change or initiate contraceptive options.  This visit will only discuss contraception options.  If you have multiple concerns or issues we ask that you schedule a comprehensive problem visit.  We prefer to do this visit in person as having a baseline blood pressure is important before initiating oral contraceptives or a pelvic exam if considering IUD.  Under special circumstances we can do this visit Telehealth - contact our front desk and we will see if this is a feasible option for you. 
Follow Up Visit (In Person):
This visit is for established patients who have been seen by Teri or Laura at Simply Women's Care needing follow-up on labs, medication management, treatment progress, or previously discussed concerns. It is not intended for new or unrelated issues. Please schedule accordingly if you have additional concerns. If you have never seen Teri or Laura at this office location, please schedule a NEW PATIENT FOLLOW UP VISIT.

Follow Up Visit (Telehealth): TN RESIDENTS ONLY
This visit is for established patients who have been seen by Teri or Laura at Simply Women's Care needing follow-up on labs, medication management, treatment progress, or previously discussed concerns. It is not intended for new or unrelated issues. Please schedule accordingly if you have additional concerns. If you have never seen Teri or Laura at this office location, please schedule a NEW PATIENT FOLLOW UP VISIT.

This visit is for established patients returning for their first follow-up after their initial hormone consultation. It is appropriate for reviewing progress after starting a treatment plan or other services initiated at your first visit. We will assess your response to treatment, review any labs if applicable, and make adjustments as needed.

Note: Please book this visit only if you have already completed your initial consultation with Laura or Teri.

This visit is for established patients returning for their first follow-up after their initial hormone consultation. It is appropriate for reviewing progress after starting a treatment plan or other services initiated at your first visit. We will assess your response to treatment, review any labs if applicable, and make adjustments as needed.

New Patient Follow Up  (Telehealth) TN RESIDENTS ONLY
This telehealth visit is for patients who have previously seen Laura or Teri at a different location and are now continuing their care at Simply Women’s Care but still need to establish as a patient at the new office location. This appointment is appropriate for follow-up on hormone replacement therapy management, or weight loss medication management.  It is not for new problems or for initial visits for hormone replacement therapy or initial weight loss medication discussions.

Note: HerMD patients establishing care with Teri or Laura at SWC, this appointment is for you!

This visit is for patients who have previously seen Laura or Teri at a different location and are now continuing their care at Simply Women’s Care but still need to establish as a patient at the new office location. This appointment is appropriate for follow-up on hormone replacement therapy management, or weight loss medication management.  It is not for new problems or for initial visits for hormone replacement therapy or initial weight loss medication discussions.
