A $50 deposit is required when booking and will be applied toward your service at your visit.
We kindly ask that any changes or cancellations be made at least 48 business hours in advance. If you cancel late, don’t show up, or arrive more than 15 minutes late, your deposit will be forfeited.
After two late cancellations or no-shows, prepayment for the full service will be required to book future appointments.

This visit includes a comprehensive evaluation by a Nurse Practitioner to assess medical necessity for Family Medical Leave (FMLA), Short-Term Disability (STD), or other employment-related medical forms.
The provider will review your medical history, symptoms, and work duties to determine appropriate recommendations. Form completion and submission are included.
Note: A separate visit fee applies for extensions, revisions, or additional employer requests.
Review and assessment to determine medical readiness to return to work after illness, injury, or procedure. Includes completion of clearance documentation.
