Appointment & Cancellation Policy

Proficient Health & Wellness is a cash-based clinic and does not bill insurance.
A valid card on file is required to book any appointment.
Appointments must be cancelled or rescheduled at least 48 business hours in advance.
Late cancellations, no-shows, or arrivals over 15 minutes late may result in a fee charged to the card on file.
All required forms must be completed before your appointment or it will be rescheduled.

By booking, you authorize applicable charges per clinic policy.

Initial Weight Loss Consultation (

This one-hour telehealth consultation is designed to help you start your weight loss journey with a personalized plan that fits your lifestyle and goals.

During this visit, we will:

Review your medical history, weight loss goals, and prior challenges
Discuss your symptoms, lifestyle factors, and expectations
Order baseline laboratory testing (lab fees are included in the initial visit)
Develop a customized care plan that may include nutritional guidance, lifestyle recommendations, and ongoing monitoring

Important Information:Laboratory testing must be completed and reviewed prior to any medication being prescribed or ordered. Based on your lab results, treatment plan, and applicable regulations, an in-person follow-up visit may be required before certain medications can be initiated or continued.

This initial visit establishes the foundation for your long-term success, with follow-up care focused on reviewing results, tracking progress, and making appropriate adjustments to your plan.
