Follow up

DOT/CDL physical for commercial drivers with guidelines set forth by the FMCSA/NRCME. 

Review of labs, urine, EKG, chest imaging, etc (as required by Surgeon). General exam in office. Sign off on Surgical Clearance form
*Patient must have ALL results available for review at time of appointment
*ANY criteria not met will not be signed off
*MUST provide a copy of the Surgeon's Pre-operative clearance request form
