We will spend 45-60 minutes Teaching about Nutrition, Metabolism, Macros, Recipes, Food items at the grocery store, the use of a Food Log, etc.
This is a good start for someone who is unsure of what Treatment choices are available and which one(s) may be best suited for them and their lifestyle.

 
Weight Loss - Follow Up (Established Patient)
HT Consult - We will review history and current symptoms, order labs, and discuss hormone medication options, such as pellets, injections, creams, patches, and tablets. 
Sex Hormones include Progesterone, Testosterone, and Estrogen.
Thyroid Hormones include TSH, T4, T3
Metabolic Labs include checking levels of: Blood Glucose, Hemoglobin A1C, Insulin, Iron, Ferritin, TSH, T4, T3, and many more.
**There is an additional $99 Monthly Hormone Management or Metabolic Management Fee that will be charged for any non-pellet options.

*Begin Phentermine therapy once BMI is confirmed. [The patient must have a Body Mass Index (BMI) of greater than 30; unless the patient has a BMI of at least 27 with other conditions/disease states, including high blood pressure (hypertension), high cholesterol (hyperlipidemia), major depression, Type 2 diabetes, pre-diabetes, insulin resistance / poly-cystic ovarian syndrome, metabolic syndrome, obstructive sleep apnea, nonalcoholic fatty liver disease, osteoarthritis.] 
*We will spend 45-60 minutes with patient to: 1) Educating patient about: Diet and Exercise, Water Intake, Daily Calorie / Carbohydrate intake, Use of Food Log, Metabolism, Food Choices, Recipes, Motivation for weight loss.
2) OARRS Review conducted. 3) Review and obtain consent. 
4) Phentermine Rx is sent to your pharmacy of choice. 
Review results so far, what's working well, what challenges have presented and how to solve those challenges. Verify that patient is still within the appropriate BMI for phentermine treatment.
Verify that patient has obtained weight loss of at least 5% of starting body weight by their 4th visit (so during the first 90 days of treatment) in order to continue phentermine treatment.
Phentermine Rx is sent to your pharmacy of choice.
Female pellet insertion. First insertion is $350, future insertions are $300 and occur approximately every 3 months. This appointment occurs after you have had your Hormone Therapy Consultation, Blood Work completed, and been approved for Pellet Therapy.
Male pellet insertion. Initial insertion is $650, future insertions are $600 and done every 6 months. 
This appointment occurs after you have had your Hormone Therapy Consultation, Blood Work completed, and been approved for Pellet Therapy.
