DOT Physical — CDL Medical Examination ($115)
Dr. Tracy is a certified FMCSA Medical Examiner. Full federal exam, vision and hearing screening, urinalysis, and Medical Examiner Certificate issued same day if cleared.

Routine and sick visits, annual physicals, chronic condition management, medication management, referrals, physicals, and forms. A $50 good faith deposit is collected at check-in for all insurance visits.

Please arrive 15 minutes early for your first visit. We require 24 hours notice for all appointment changes. A cancellation fee applies to late cancellations, late reschedules, and no-shows: $75 for new patients and $50 for established patients, charged to the card on file the same day.
